From 2020 we will be providing the Diabetic Health Clinic Lifestyle Program in a video format so that a minimally trained facilitator can run the program at any location. As with all of our venue programs, everyone who attends will be attending for no cost. Now that is an idea whose time has come! 

If you would like to be a DHC facilitator please complete the form so that we can provide you with additional information. After you have comepted the form we will call you to discuss where you would like to run the program. We will also provide you with additional information about the program, how it works and what a faciltator needs to do.
